Cranio-mandibular dysfunction (TMD) – What is it?

Cranio-mandibular dysfunctions (TMD), dysregulation of the temporomandibular joint and the masticatory system, usually develop over a long period of time, initially unnoticed and one day audible or palpable or even painful. The causes are rarely only in the oral and maxillofacial area.

Causes and symptoms of TMD

The symptoms of TMD are diverse and affect not only the jaw, but also the entire body. The most common complaints include pain in the area of the masticatory muscles and the temporomandibular joints (myofacial pain), headaches, tinnitus, neck tension and even dizziness. These symptoms often occur insidiously and are often wrongly attributed to other causes.

Almost 10% of the population is affected by a disease of the masticatory system

There are different forms of diseases of the temporomandibular joint, which differ as follows:

  • Occlusopathy, starting from the teeth and the incorrect teeth
  • Myopathy, starting from the masticatory muscles
  • Arthropathy, originating from the temporomandibular joint and its structures, e.g. in rheumatism, arthritis or after an accident

About 10% of the population suffer from a disease of the masticatory system. Women are affected more often than men. Younger patients more often than older patients.

The reasons for this TMJ disorder can have the following causes, for example:

  • too much stress
  • Consequences of orthodontic treatment

  • Psychological stress

Our dentists will first have a detailed discussion with you. On the basis of an instrumental and clinical functional analysis of the patient and models, a temporomandibular joint measurement and analysis as well as an examination of the teeth, we can detect a disorder in the masticatory system.

Through X-rays and, if necessary, supplementary 3D X-rays, we can precisely diagnose structural causes.

The right diagnosis: empathy and experience are crucial

At the beginning of the treatment, a detailed discussion without time pressure is crucial, in which the development of the disease is discussed and the patient’s expectations are compared with the therapeutic options. Due to the complex interconnectedness in the body, the diagnosis and treatment of TMD therefore requires many years of training and experience as well as special empathy. The most important “instruments” are eyes, ears and sensitive hands.

Diagnostic procedures and therapeutic approaches

Often, complex jaw measurements, MRI and X-ray examinations of the temporomandibular joints are just as superfluous for TMD diagnostics as extensive reconstructive measures (crowns) for therapy afterwards. If special instrumental procedures are put in the foreground (e.g. DIR, PlaneSystem, Freecorder), patients should be careful, because the decisive factor is the skill of the dentist, not the equipment.

Holistic TMD therapy at the CMD Center Munich-Nymphenburg

At the CMD Center Munich-Nymphenburg of Dr. Wolf-Dieter Seeher, TMD patients are not only diagnosed and treated dentistically, but holistically. Not only the usual orthopaedic, physiotherapeutic and osteopathic methods are used, but particular emphasis is placed on finding the cause and a corresponding therapy that is as causal as possible.

In doing so, we rely on gentle and reversible treatment approaches. As a rule, affected patients can be prescribed an occlusal splint. Concomitant therapies can include analgesic medications, TENS (transcutaneous electrical nerve stimulation), trigger point infiltrations, as well as autogenic training, physiotherapy and osteopathy, depending on the requirements.

In the case of severe jaw pain, we first use water-filled bite cushions, so-called “Aqualizer”, for relaxation and also prescribe relaxing painkillers. Aqualizers should only be worn for a few weeks. As soon as the acute symptoms have subsided, we start with further measures such as functional analysis and impressions. The occlusal splint is only adjusted after successful pain therapy.

Lengthy treatment process with individual care

Sometimes symptom or pain relief succeeds in the short term, in other cases the treatment is a lengthy, subtle process that in a few cases cannot be brought to a successful conclusion.
